Adult Care Management – Health Home

Home-based care management services for adults with chronic mental health, medical and substance abuse disorders. Services include a health risk assessment, care development/planning, crisis intervention, advocacy, coordination and linkage to medical, substance abuse and mental health providers, and housing, legal, benefit, vocational and educational services.
