OnTrackNY supports young people who may be impacted by changes in their thinking, hearing or seeing things that others don’t, disruptions in school or work, or pulling away from people and activities they care about. With a focus on early intervention, we serve young people ages 16-30 in Suffolk County within two years of first experiencing these changes.
With a community-based focus, our team employs evidence-based practices to help youth achieve their goals, prevent disability, and reduce hospitalization rates, prioritizing shared decision-making and offering flexible and accessible services. FSL’s committed team of professionals can operate within the community, providing a range of services, including screening, psychiatric assessments, therapy, crisis intervention, and peer support.
